New Patient Registration
You can register as a new patient by visiting the practice reception.
If you visit reception, the receptionist will give you a form to complete, you will need to provide details of your previous address and GP. It is also helpful if you can provide your NHS number.
You will be registered as a patient of the practice and will be allocated a named GP. You can request a preference of practitioner and a note will be made of this request in your record. When you have registered you will receive a letter from patient services confirming your registration. You will be able to make an appointment with any GP.
If you wish to book an appointment for a 'New Patient Check' please speak to reception. If you are on any regular medication we would advise that you obtain a prescription from your current GP whilst we are processing your registration and also attach a copy of your regular medication to your registration form.
Our list is open to patients living in the Highgate and Tinsley area. Existing patients moving just outside of the boundaries or new patients who move in with existing patients just outside of the boundaries may be accepted at the discretion of the practice.
Change of Name, Address or Contact Details
We ask that you notify us of any change to your address, telephone numbers or any other contact details, such as a change of name. We can only use these current details, so if they have not been updated then we may not be able to contact you when necessary.
